What Is Microdermabrasion?

The Science
Microdermabrasion is one of those treatments that sounds simple. “Just Exfoliation, ”but biologically it’s doing something more aggressive and structured to the skin barrier than people often realize.
Microdermabrasion is a controlled mechanical exfoliation that uses either crystal abrasion (aluminum oxide crystals) or a diamond-tipped wand with suction to physically remove the outermost layer of the stratum corneum (dead keratinized skin cells). (American Academy of Dermatology Association, n.d.).
The top layer is not just “dead skin”; it is part of your barrier system, responsible for:
Transepidermal water loss (TEWL) regulation
Microbiome protection
Defense against irritants and pathogens
Maintaining lipid balance (ceramides, cholesterol, fatty acids)
So when you abrade it, you’re not just polishing the surface; you are temporarily compromising a functional protective layer.

Benefits of Microdermabrasion
When used appropriately, research and clinical dermatology reviews show microdermabrasion can help with:
Mild acne comedones (blackheads/whiteheads)
Superficial hyperpigmentation (especially epidermal level)
Uneven texture and dullness
Mild photodamage
Enhancing penetration of topical products temporarily
Microdermabrasion can also stimulate a mild wound-healing cascade, increasing epidermal turnover and collagen signaling over time. (Fabbrocini et al., 2014).
So microdermabrasion can be used, but as a tool with a narrow ideal use case.
The Downside of Microdermabrasion
Mechanically stripping the stratum corneum triggers:
Increased transepidermal water loss (TEWL)
Temporary inflammation
Disruption of lipid barrier structure
Increased skin sensitivity post-treatment
In sensitive or reactive skin types, this can translate into:
Redness that lingers longer than expected
Breakouts from inflammation (not “purging”)
Delayed barrier recovery
Post-inflammatory hyperpigmentation (PIH) in prone skin tones
Heightened reactivity to skincare products afterward
In the dermatology literature, repetitive or overly frequent mechanical exfoliation is associated with barrier impairment and increased susceptibility to irritants, especially in already compromised skin. (Fabbrocini et al., 2014).
Frequency Matters
Clinically, microdermabrasion is generally recommended at:
Every 2–4 weeks
That timing matters because the epidermis needs time to:
Rebuild lipid layers
Normalize barrier function
Resolve micro-inflammation
Shortening that window can push skin into a cycle of constant repair instead of recovery.
Better in Targeted Areas
For many people, especially those with sensitive or acne-prone skin, microdermabrasion tends to be more tolerable when limited to:
Nose (sebaceous filaments/blackheads)
Between eyebrows (congestion zone)
Chin (comedonal buildup)
These areas are naturally more sebaceous and resilient compared to:
Cheeks (thinner barrier, more reactive)
Perioral area (high sensitivity zone)
Full-face abrasion can be unnecessary if the concern is localized congestion.
Sensitive Skin Reality
For sensitive, reactive, or barrier-compromised skin types, microdermabrasion can behave less like “gentle exfoliation” and more like repetitive mechanical stress.
Especially when:
There is active or recent acne inflammation
The barrier is already impaired
There is a history of PIH
Skin is reactive to friction, tools, or metals
In those cases, the skin does not always respond with “glow”; it responds with inflammation first, then repair delay.
Personal Experience with Microdermabrasion
I started dealing with a stubborn breakout on my cheek that never really resolved the way I expected. Instead of healing cleanly, it lingered, turned into lingering marks, and eventually led to scarring and hyperpigmentation that took months to fade. Around the same time, I also started experiencing hormonal acne for the first time in my late 20s, which felt like my skin completely changed overnight.
What I didn’t understand then, but do now, is that the skin on the cheek is naturally more delicate than other areas of the face. It’s thinner, more reactive, and more prone to post-inflammatory hyperpigmentation. It also doesn’t tolerate repeated mechanical stress very well, especially when the skin barrier is already compromised.
During that period, I was also receiving frequent in-school treatments and hands-on practice, and I now realize my skin was subjected to repeated physical exfoliation and disruption before it had time to fully recover. Instead of supporting my barrier, I was unintentionally putting it under constant stress.

So, when I think about microdermabrasion from my own experience, it makes sense that my skin reacted the way it did. If it was done repeatedly during a time when my barrier was already inflamed and trying to heal, it likely contributed to a cycle I didn’t recognize at the time: more inflammation, slower healing, and marks that took much longer to resolve.
I noticed that my skin would feel overly sensitive afterward, almost like “sandpaper on the face.” At the time, I didn’t have the language for it, but what I was really experiencing was acute disruption of my stratum corneum. The skin barrier was being mechanically stressed beyond what my skin could comfortably tolerate.
In hindsight, that combination of hormonal acne, reactive cheek skin, and repeated physical exfoliation created a perfect storm for prolonged inflammation and delayed barrier recovery. It’s taken time for my skin to calm down and feel like itself again, and it’s also changed how I think about treatments marketed as universally safe or beneficial.
Now I understand that my skin tends to respond better to gentler, barrier-supportive approaches and that what works well for some skin types can actually be too aggressive for mine when it’s already in a sensitive or healing state.
Balanced Truth
It’s important to hold both truths:
Microdermabrasion is:
Useful for resilient, thicker, congestion-prone skin
Helpful for light texture refinement
Effective when used infrequently and strategically
But can be:
Overly disruptive for sensitive or barrier-impaired skin
Counterproductive when overused
Problematic when applied to inflamed or healing acne regions
Bottom line
Microdermabrasion is not inherently harmful, but it is mechanically aggressive relative to what sensitive, acne-recovering skin often needs, especially on the cheeks.
In those cases, the skin usually responds better to:
Barrier repair (ceramides, cholesterol, fatty acids)
Low-irritation chemical exfoliation (if any, very sparingly)
LED therapy or gentle enzymatic exfoliation instead of abrasion
